Fatal toxic leukoencephalopathy secondary to overdose of a new psychoactive designer drug 2C-E ("Europa").

A fatal case of toxic leukoencephalopathy occurred after someone ingested the new psychoactive designer drug 2C-E, also called "Europa." Recreational hallucinogenic drugs are growing in popularity, partly because information and newer designer substances are easily available online without legal consequences. Magnetic resonance imaging (MRI) showed selective diffuse toxic injury to the cerebral white matter, while the cortex and most deep gray nuclei were spared. This is the first reported description of MRI findings likely related to a lethal ingestion of 2C-E.

Access to information on acquiring and synthesizing newer designer drugs has increased, largely via the Internet. As these drugs become more common, laboratory methods for identifying them have improved. This case describes an overdose of the uncommon designer drug 2C-E and explains the basics of liquid chromatography and mass spectrometry, two techniques used to isolate and identify the drug. These laboratory methods, though often overlooked, can be crucial for diagnosing and managing certain patients.

Near-death experiences in medicine.

Near-death experiences (NDEs) occur in 10% to 20% of patients who come close to death, involving vivid phenomena such as out-of-body experiences, pleasant feelings, seeing deceased relatives, a life review, or conscious return into the body. Alternate psychological and physiological theories exist but face methodological and neuroscientific limitations. Counselors trained with NDE literature have reduced suicidal thoughts, bereavement, and posttraumatic stress disorder in their patients. Patients who have had an NDE show significant transformation in spiritual and emotional lives, reporting renewed meaning, existential awareness, and mystical experiences. Family and friends also report increased comfort, hope, and inspiration. NDEs positively impact medical practice and represent a growing paradigm shift beyond naturalistic interpretations.
