Involuntary MDMB-4en-PINACA intoxications following cannabis consumption: clinical and analytical findings

A synthetic cannabinoid called MDMB-4en-PINACA, found in low-THC cannabis products, caused severe intoxication in 13 patients between November 2020 and March 2021. All patients had altered mental status; other symptoms included seizures (5 cases), vomiting (6), paranoia (6), hallucinations (3), and headaches (3). Patients typically recovered to normal or near-normal after about 11 hours of observation. No deaths occurred. Toxicological analysis confirmed MDMB-4en-PINACA in urine, blood, or drug material samples from all patients, indicating the substance can cause or contribute to various clinical disorders.
