Among young cannabis users, a brief psychotic disorder may be either acute cannabis psychosis or an early sign of schizophrenia. Clinicians lack clear criteria to distinguish these conditions, which have different prognoses and treatments. Since 2010, hair concentrations of THC and CBD were measured in 256 New Caledonian cannabis consumers with various mental disorders. At initial psychiatric consultation, a 3 cm proximal hair sample was analyzed by LC-MS/MS. After six months of follow-up, four diagnostic groups were identified: control, acute cannabis psychosis, chronic psychosis, and other personality disorders. A high hair THC level (> 0.7 ng/mg) may mark chronic psychosis, especially with a low CBD/THC ratio (< 0.26).

Among young cannabis users, a brief psychotic disorder may signal acute cannabis psychosis or the onset of schizophrenia, but no clinical or paraclinical criteria currently differentiate these conditions. Researchers measured THC and CBD concentrations in head hair from 256 New Caledonian cannabis consumers at initial psychiatric consultation and followed them for six months. A high hair THC level (>0.7 ng/mg) combined with a low CBD/THC ratio (<0.26) distinguished those who developed chronic psychosis. Higher CBD/THC ratios (>0.43) characterized acute cannabis psychosis, while ratios below 0.32 marked other personality disorders. The findings suggest that hair cannabinoid profiles, alongside clinical evaluation, could aid early diagnosis and management of brief psychotic disorder.

In a study of 256 New Caledonian cannabis users, delta-9 tetrahydrocannabinol (THC) and cannabidiol (CBD) concentrations in head hair were measured at initial psychiatric consultation and after a 6-month follow-up. A high hair THC level (>0.7 ng/mg) combined with a low CBD/THC ratio (<0.26) emerged as a potential marker for chronic psychosis. For acute cannabis psychosis, the CBD/THC ratio was higher (>0.43) than for other personality disorders (<0.32). The findings suggest CBD may protect against THC's harmful effects, and hair cannabinoid analysis could aid psychiatric diagnosis and early management.
