[Suicidal ideation and behaviors: a transdiagnostic dimension for precision psychiatry].

Suicidal thoughts and behaviors form a distinct dimension that cuts across psychiatric diagnoses, supported by genetic, biomarker, and preclinical evidence. Key endophenotypes include impulsivity, anhedonia, and hopelessness, with emerging mechanisms in inflammation, stress response, serotonin function, and neuroplasticity. Treatments like lithium and ketamine target these pathways. The French Minds cohort within the PEPR PROPSY project will use multimodal approaches to identify biological signatures and trajectories of this dimension.

Abstract

Suicidal thoughts and behaviors represent a transdiagnostic dimension independent of traditional psychiatric diagnoses. This specificity is supported by genetic studies, biomarkers, and preclinical research on endophenotypes associated with suicidal behaviors, such as impulsivity, anhedonia, and hopelessness. Clear mechanisms involving inflammation, the stress response, the serotonergic system, and neuroplasticity are emerging. Specific treatments, such as lithium and ketamine, target these mechanisms. The French Minds cohort of the PEPR PROPSY will identify the biological signatures and trajectories of this dimension using comprehensive multimodal approaches.
