Acute behavioural disturbance associated with phenibut purchased via an internet supplier.

Two cases of phenibut toxicity after internet purchase are reported. A 20-year-old female presented with decreased consciousness and delirium after using phenibut the prior day; she recovered fully over 24 hours with supportive care. Plasma phenibut concentration was 29.7 μg/ml. A 38-year-old male presented with agitated delirium after using tetrahydrocannabinol, alcohol, and phenibut; he required intubation for airway protection after suboptimal response to sedation. His plasma phenibut concentration was 36.5 μg/ml. Altered mental status was the predominant manifestation. Clinicians should be aware that internet access widens availability of such substances.
